Lead agencies must conduct an inspection of each surface mining operation within six months of receiving its annual report, and in no event less than once in any calendar year. These inspections must be conducted using a form developed by the Department of Conservation and approved by the State Mining and Geology Board (SMGB).
The Office of Mine Reclamation has updated the Surface Mining Inspection Report form (MRRC-1). The newly revised form was approved by the SMGB at its July 2013 meeting and took effect July 17, 2013. In addition, OMR has developed a convenient Lead Agency Inspection Notice Form to assist lead agencies with PRC 2774(b) requiring lead agencies submit a notice with the MRRC-1 Inspection Form.
Annual Reports are required from all mines subject to SMARA. Regardless of the status of your mining operation, an Annual Report and payment for the applicable reporting fees must be submitted on an annual basis until reclamation is certified complete by both your lead agency and the Office of Mine Reclamation. In order for an Annual Report to be considered complete and timely, the law requires all information requested on the form be submitted along with payment for the reporting fee. A copy of the Annual Report must be filed with the respective lead agency by July 1, 2016. For convenience, the Office of Mine Reclamation posts the updated Annual Report forms online during May of each year.
Online Annual Reporting System
The Office of Mine Reclamation is excited to announce the launch of online submissions for the Mining Operation Annual Reports (MRRC-2). OMR has constructed an online reporting system which will assign a unique user code to each California Mine Identification number required to file an Annual Report. Participants for the 2015 pilot program have already been selected. For your convenience, the OMR will be moving toward the online submittal of all annual reports and associated reporting fees beginning with the 2016 Reporting Year, due July 1, 2017.

The Office of Mine Reclamation maintains a database of information submitted annually by mine operators in the State. Because this office derives this information from individual operators, we cannot guarantee its accuracy. Our Mines Online map is refreshed nightly from this database and provides downloads of selected data.
